Welcome to our website. Physical Rehabilitation Network, LLC, a California limited liability company (“us”, “we”, “PRN”), owns and operates www.proactivecolorado.com (the “Site”) as a service to our affiliates and our network of physical therapists and their patients.

The information and content on the Site or available through the Site, including, without limitation, text, design, graphics, logos, icons, images, video and audio clips, downloads, interfaces, code and software, as well as the selection and arrangement thereof (collectively, “Content”) is for your general educational information only. The Content cannot, and is not intended to, replace the relationship that you have with your health care professionals. The Content on this Site should not be considered medical advice and is not intended as medical advice. If you are experiencing a medical emergency, you should not rely on any information on this website and should seek appropriate emergency medical assistance, such as calling “911”. You should talk to your health care professionals for diagnosis and treatment, including information regarding which medicine or treatment may be appropriate for you. None of the Content on this Site represents or warrants that any particular medicine or treatment is safe, appropriate or effective for you.

1. Agreement. This Agreement contains the terms and conditions upon which you (“you” or “the User”) may access and use the information, products and services available through the Site. By using or visiting the Site or any of the products or services offered through the Site, you expressly agree to be bound by this Agreement and to follow the terms of this Agreement and all applicable laws and regulations governing the Site. If you do not agree to this Agreement, or to the Privacy Policy, all of which are incorporated by reference herein, do not use the Site or any products or services offered through the Site.

We reserve the right at any time to: (i) change the terms and conditions of this Agreement; (ii) change the Site, including eliminating or discontinuing any content or feature of the Site; or (iii) change any fees or charges associated with any products and services available through the Site. We may modify this Agreement from time to time without notice to you. You agree to review this Agreement periodically to ensure that you are aware of any modifications. Your continued access or use of the Site shall constitute your agreement to be bound by the modified terms and conditions of this Agreement.

This Agreement will remain in full force and effect as long as you are a user of the Site and in the event of termination of any membership, service or feature, you will still be bound by your obligations under this Agreement, including any indemnifications, warranties and limitations of liability.

2. Privacy. Your visit to the Site is also governed by our Privacy Policy, which should be reviewed at www.proactivecolorado.com/privacy-policy.

3. Use of Content on the Site. All Content on the Site or available through the Site is the exclusive property of and owned by PRN, its licensors or its content providers, and is intended to educate and inform you about events, products and services offered or described on the Site. The Content is protected by copyright, patent, trademark and other applicable laws. Nothing contained in this Agreement shall be deemed to grant to you or any other user any rights, title, or interest in or to any copyright, patent, trademark, or other proprietary right of PRN, its licensors or its content providers.

Subject to your compliance with this Agreement, you may access, copy, download and print one (1) copy of any materials displayed on the Site for your personal and non-commercial use, provided that you do not modify or delete any copyright, trademark or other proprietary notice that appears on the materials you access, copy, download or print. Any other use of such materials, including but not limited to the modification, distribution, transmission, performance, broadcast, publication, uploading, licensing, reverse engineering, transfer or sale of, or the creation of derivative works from, any material, information, software, products or services obtained from the Site, or use of the Site for purposes competitive to us and our affiliates, is expressly prohibited without our prior written consent. You may not use any hardware or software intended to damage or interfere with the proper working of the Site or to surreptitiously intercept any system, data or personal information from the Site. You agree not to interrupt or attempt to interrupt the operation of the Site in any way. Use of the Site and Content for any purpose other than as expressly authorized in this Agreement is a violation of our copyrights and other proprietary rights, and is strictly prohibited.

You acknowledge and agree that you do not acquire any ownership rights by using the Site or the Content. PRN, or its licensors or content providers, retain full and complete title to the Content, including all associated intellectual property rights, and provide the Content to you under a license that is revocable at any time in our sole discretion. We reserve the right, in our sole discretion, to limit or terminate your access to or use of the Site at any time without notice. Termination of your access or use will not waive or affect any other right or relief to which we may be entitled at law or in equity. We neither warrant nor represent that your use of Content on the Site will not infringe rights of third parties not affiliated with us. We also do not guarantee that technical difficulties will not occur during the download of Content or that the Content will download successfully.

4. Content Submitted by You. Certain portions of this Site may be configured to permit users to submit content. You acknowledge and agree that you are responsible for any content you may submit via the Site, including but not limited to any text, images, photos, audio, video, and all other forms of data or communication that a user provides or submits to us or our affiliates in any way including but not limited to social media, Linked In, Facebook, Twitter, survey responses, reviews and contests (collectively, “User Content”), including the legality, reliability, appropriateness, originality and copyright of all such User Content. By sending or transmitting User Content to us, or by posting such User Content to the Site, you hereby grant, and represent, and warrant that you have all rights and authority necessary to grant, (i) us and our designees, a perpetual, irrevocable, non-exclusive, transferable, royalty-free, fully sub-licensable, fully paid-up, worldwide license and right to use, modify, copy, reproduce, publish, distribute, publicly perform or display, adapt, translate, create derivative works, archive, store, upload, share, post, sublicense, or otherwise make available, such User Content in any form and through any media of any kind now known or later developed, for any purpose, with or without attribution, and (ii) subject to the restrictions set forth in this Agreement, all users a perpetual, irrevocable, non-exclusive, royalty-free license and right to use, such User Content for each such user’s personal, non-commercial use. If we decide, in our sole discretion, to attribute User Content to you, you hereby grant us the right to use your name with respect to such attribution, and waive any claims (including, without limitation, any privacy or publicity rights claims) with respect to such use of your name. You hereby irrevocably waive and assign to us any and all moral rights you may have in or with respect to any User Content you provide to us. You further agree that we are under no obligation to post, display or otherwise use any User Content, and we have no obligation whatsoever to pay you any royalty or other amounts on any revenues or other consideration that we receive directly or indirectly from the use or display of your User Content or otherwise from the exercise of our rights granted under this Agreement.

You represent and warrant that: (i) you own, or have the necessary licenses, rights, consents, and permissions to the User Content and to grant the rights and licenses to us under all patent, trademark, trade secret, copyright or other proprietary or intellectual property rights in and to any and all User Content in the manner contemplated by the Site and this Agreement; and (ii) you have the written consent, release, and/or permission of each and every identifiable individual person in the User Content to use the name or likeness of such person in the manner contemplated by the Site and this Agreement or, if such persons are minors, the written consent, release, and/or permission of such minor’s parent or legal guardian.

You acknowledge and agree that we have the right, but not the obligation, to monitor the Site and the User Content that you or third parties transmit or post; to alter or remove any such User Content; to disclose such User Content and the circumstances surrounding their transmission to any third party in order to operate the Site properly; to protect ourselves, our sponsors and our users; and to comply with legal obligations or governmental requests. We do not guarantee the accuracy, integrity, or quality of the User Content. Nonetheless, we reserve the right, at all times and for any reason whatsoever, to edit or remove any User Content, as well as selectively publish certain User Content (such as positive User Content).

5. Restrictions on User Content and Your Use of the Site. Without limiting the generality of any other provisions of this Agreement, you agree that you will not post, upload, or transmit any User Content that: (i) is offensive, threatening, illegal, harmful, tortious, abusive, defamatory, obscene, vulgar, hateful, that infringes any third party right, or is otherwise objectionable; (ii) promotes illegal activities or conduct that is offensive, threatening, harmful, tortious, abusive, defamatory, obscene, vulgar, hateful, or is otherwise unlawful; (iii) is false, misleading, or defamatory; (iv) is protected by copyright, trade secret, or subject to any other third party intellectual property rights or privacy rights unless you are the owner of such rights or you have the express permission from the rightful owner of such rights to upload, post, or submit such content and to grant us all the rights granted herein; (v) provides personal data such as telephone numbers, home or e-mail addresses, full names or last names unless you have the explicit permission of the subject of such personal data (including any photos of any children under the age of 18 without the express permission of the child’s parent or guardian); (vi) involves the transmission of unsolicited mass mail, spam, chain mail, junk mail, pyramid schemes, or otherwise interferes with other users’ enjoyment of the Site; (vii) falsely states or implies that such content is sponsored or endorsed by us or our affiliates; (viii) contains restricted or password-only access pages or hidden pages or images; (ix) involves commercial activities, advertising or sales without our express prior written consent; (x) or involves sending or otherwise making available any material that contains viruses, defects, Trojan horses, worms, time bombs, or any other similar software that may damage the operation of PRN’s, its affiliates’ or any third party web provider’s servers, computers, or networks.

You further agree that you will not: (i) use the Site for any unlawful purpose; (ii) restrict or inhibit any other User from using the Site, including, without limitation, by means of “hacking” or defacing any portion of the Site; (iii) impersonate any person or entity, whether actual or fictitious, including any of our employees or representatives; (iii) express or imply that any statements you make are endorsed by us, without our prior written consent; (iv) modify, adapt, sub-license, translate, sell, reverse engineer, decompile or disassemble any portion of the Site or otherwise attempt to derive any source code or underlying ideas or algorithms of any part of the Site; (v) engage in spamming or flooding; (vi) remove any copyright, trademark or other proprietary rights notices contained on the Site; (vii) “frame” or “mirror” any part of the Site; (viii) use any robot, spider, offline reader, site search/retrieval application or other manual or automatic device or process to retrieve, index, data mine or in any way reproduce or circumvent the structure or presentation of the Site or its contents without our prior written consent; or (ix) engage in any other unlawful or unreasonable, in our sole discretion, use of the Site or Content.

6. Links. The Site may contain links to other websites, which may be owned or operated by us, an affiliate or third parties. We do not approve, recommend or endorse the content of any third party websites. We are not responsible for the content of linked third party sites or any products or services that may be offered through such sites, or the privacy or other practices of such sites, and we do not make any representations regarding their content or accuracy. We provide these links only as a convenience and, therefore, we encourage you to read and become familiar with the terms of use and privacy policies associated with any linked site. Your use of third party sites, including, without limitation, your submission of content to such sites and payment of services through such sites, is at your own risk and subject to the terms and conditions of use for such sites. We are not responsible for any losses, damages or other liabilities incurred as a result of your use of any linked sites.

7. Site Use. We grant you a limited, revocable, nonexclusive license to use the Site solely for your personal use and not for any commercial use. Your use of the Site is subject to the terms and conditions of this Agreement, as may be amended from time to time, and all applicable laws and regulations. Without limiting the generality of any other provisions of this Agreement, you may not: (i) use any device, program, algorithm, or methodology, or any manual process to access, acquire, copy, or monitor any portion of the Site or obtain or attempt to obtain any materials, documents, or information through any means not purposely made available through the Site; (ii) attempt to gain unauthorized access to any portion of the Site or any content on the Site, or any systems or networks connected to the Site, by hacking, password “mining”, or any other illegitimate means; (iii) reverse look-up or trace any information of any other Site user or otherwise use the Site for the purposes of obtaining information of any other Site user; (iv) use any device, software, or process to interfere with, or attempt to interfere with, the proper working of the Site or any content on the Site, or any systems or networks connected to the Site, or with any other person’s use of the Site; (v) remove any copyright, trademark, or other proprietary rights notice contained in or on the Site; (vi) use the Site or any content on the Site for any purpose that may directly or indirectly be competitive or potentially competitive with us or any of our affiliates or network providers; or (vii) use the Site or any content on the Site for any purpose that is unlawful or prohibited by this Agreement. The use of this Site is at our sole discretion. In addition to exercising other remedies that may be available, we may, at any time, refuse or terminate your account or suspend or prohibit your access to the Site without prior notice to you for breaching this Agreement or for any other reason whatsoever.

8. Registration. You may browse the Site and view content without registering, but as a condition to using certain aspects of the Site, you may be required to register and select a password and user name (“User ID”). You shall provide accurate, complete, and updated registration information. Failure to do so shall constitute a breach of this Agreement, which may result in immediate termination of your account.

Each time you use a User ID, you will be deemed to be authorized to access and use the Site in a manner consistent with the terms and conditions of this Agreement, and we have no obligation to investigate the authorization or source of any such access or use of the Site. Without limiting the generality of any other provisions of this Agreement, you may not: (i) select or use as a User ID a name of another person with the intent to impersonate that person; (ii) use as a User ID a name subject to any rights of a person other than you without appropriate authorization; or (iii) use as a User ID a name that is otherwise offensive, vulgar or obscene. We reserve the right to refuse registration, cancel a User ID or deny access to the Site in its sole discretion. We may refuse to grant you a user name that impersonates someone else, is or may be illegal, is or may be protected by trademark or other proprietary rights law, is vulgar or otherwise offensive, or may cause confusion, as determined by us in our sole discretion.

Your user name and password are personal to you. You are solely responsible for any and all activities (including but not limited to purchases) that are conducted through your account and you shall be responsible for the confidentiality and use of your User ID. You agree not to (i) transfer or resell your use of or access to the Site to any third party or (ii) use another user’s account without such other user’s express permission. We are not liable for any harm caused or related to the theft or misappropriation of your user name or password, disclosure of your user name or password, or your authorization of anyone else to use your user name or password. You shall immediately notify us of any unauthorized use of your password or user name or any other breach or threatened breach of this Site’s security. If you believe that your account with us is no longer secure, you must promptly change your password and immediately notify us of the issue at compliance@prnpt.com. Please read our Privacy Policy, which describes the personally identifiable information we collect, use, disclose, manage and store.

9. Purchases. Our Site may provide links to third-party service providers to process transactions on our behalf or on behalf of our network providers (“Transaction(s)”). Any third-party service providers will have their own terms of use and/or privacy policies, which you may review on their websites. If you wish to pay for services online, you will be asked to supply certain information applicable to your Transaction, including, without limitation, credit card and other information. You agree that all information that you provide in connection with your Transaction will be accurate, current and complete. You agree to pay all charges incurred by you or any users of your account and credit card (or other applicable payment mechanism). You acknowledge that complying with applicable laws is your responsibility, and YOU AGREE NOT TO HOLD PRN LIABLE FOR YOUR FAILURE TO COMPLY WITH ANY LAW OR OUR FAILURE TO NOTIFY YOU OF, OR PROPERLY APPLY, ANY LAW. WE WILL COMPLY WITH LAW ENFORCEMENT AND MAY PROVIDE THEM WITH ALL INFORMATION YOU SUBMIT TO US TO ASSIST IN ANY INVESTIGATION OR PROSECUTION THEY MAY CONDUCT.

You represent and warrant that all information you provide, including but not limited to all information concerning your name, address, credit card number, and other identifying information of any nature will be true, complete and correct, and that you will update all information as it changes. You agree that you will only use credit cards belonging to you. You further agree that you will not attempt to conceal your identity by using multiple IP addresses or email addresses to use or to make Transactions on or through the Site.

10. Trademarks and Copyrights. Trademarks, logos and service marks displayed on the Site are registered and unregistered trademarks of PRN, its licensors or content providers, or other third parties. All of these trademarks, logos and service marks are the property of their respective owners. Nothing on the Site shall be construed as granting, expressly or implied, any license or right to use any trademark, logo or service mark displayed on the Site without the owner’s prior written permission. We reserve all rights not expressly granted in and to the Site and the Content. The trademarks, logos and service marks owned by us, whether registered or unregistered, may not be used in connection with any product or service that is not offered by us, or in any manner that disparages PRN or its affiliates. Misuse of any trademarks, logos or service marks is prohibited. The Site and the Content, including but not limited to text, design, graphics, interfaces and code, and the selection and arrangement thereof, is protected as a compilation under the copyright laws of the United States and other countries.

11. Accuracy, Completeness and Timeliness of Information on the Sites. We are not responsible if information made available on the Site is not accurate, complete or current. The Content on the Site is provided for general information only and should not be relied upon or used as the sole basis for making decisions without consulting primary, more accurate, more complete or more timely sources of information. Any reliance on the Content is at your own risk. We reserve the right to modify the Content at any time, but we have no obligation to update any information on the Site. 12. Intended Audience. This website is intended for adults only, 18 years of age and older. This website is not intended for any children under the age of 13. 13. Indemnification. You agree to indemnify, defend and hold PRN and its directors, officers, shareholders, partners, employees, affiliates, licensors, suppliers, advertisers, sponsors, successors, and assigns harmless from and against any demands, liability, loss, claims, damages and expenses (including reasonable attorney’s fees and costs) made against us due to, arising out of, or related to (i) your access to the Site, (ii) your use of the Site, (iii) any User Content posted, uploaded or transmitted by you, (iv) your use or distribution of any User Content, (v) your breach of this Agreement, (vi) any use of your user name or password by you or any third party, (vii) the infringement or other violation by you, or any third party using your account or user name, of any intellectual property or other right of any person, or (viii) your breach of this Agreement. We reserve the right, at our own expense, to assume exclusive defense and control of any matter otherwise subject to indemnification by you and, in such case, you agree to cooperate with us in the defense of such matter.

14. Disclaimer of Warranty. YOUR USE OF THE SITE, ALL CONTENT, USER CONTENT, AND ALL PRODUCTS AND SERVICES ACCESSIBLE OR AVAILABLE THROUGH THE SITE IS AT YOUR SOLE RISK. THE SITE, ALL CONTENT, USER CONTENT, AND ALL PRODUCTS AND SERVICES ACCESSIBLE OR AVAILABLE THROUGH THE SITE ARE PROVIDED OR MADE AVAILABLE ON AN “AS IS,” “AS AVAILABLE” BASIS. TO THE EXTENT YOUR JURISDICTION DOES NOT ALLOW LIMITATIONS ON WARRANTIES, THIS LIMITATION MAY NOT APPLY TO YOU.

15. Limitation of Liability. YOUR SOLE AND EXCLUSIVE REMEDY FOR DISSATISFACTION WITH THE SITE OR CONTENT OR ANY LINKED SITE IS TO STOP USING THE SITE, CONTENT, OR LINKED SITE, AS APPLICABLE. 17. Applicable Law. This Agreement shall be governed by and construed in accordance with the laws of the State of California, without regard to its provisions relating to conflicts of law. You agree that any legal action or proceeding between you and us for any purpose concerning this Agreement or the parties’ obligations hereunder shall be brought exclusively in a court of competent jurisdiction sitting in San Diego, California, United States of America. In light of the nature of this Agreement, you understand and agree that money damages may be insufficient to rectify a breach and that, consequently, we will be entitled to seek equitable relief upon a breach of the Agreement by you.

18. Severability. If any provision of this Agreement shall be adjudged by any court of competent jurisdiction to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary so that this Agreement will otherwise remain in full force and effect.

19. Waiver. Any failure by us to enforce or exercise any provision of this Agreement shall not constitute a waiver of that right or provision. Our failure to act with respect to a breach by you or others does not waive our rights to act with respect to subsequent or similar breaches.

20. Entire Agreement. This Agreement, including the policies incorporated herein by reference, comprises the full and final understanding between you and us with respect to the subject matter thereof and merges and supersedes any and all other agreements, understandings or representations, written or oral, with respect to the subject matter hereof. This Agreement shall be binding upon and inure to the benefit of the parties hereto and their respective successors and lawful assigns.

21. Termination. This Agreement shall remain effective until terminated in accordance with its terms. We may terminate this Agreement and your access to and use of the Site at any time, with or without notice, for any reason, which may result in the forfeiture and destruction of all User Content associated with you. All provisions of this Agreement that by their nature survive termination shall survive termination, including, without limitation, ownership provisions, warranty disclaimers, indemnity, and limitations of liability.

Wes has been practicing physical therapy for over 12 years in the outpatient orthopedic setting. He founded the Aurora-Southlands branch of Pro Active Physical Therapy in 2009. Wes received his undergraduate degree from Ohio Wesleyan University in 2001. While at Ohio Wesleyan he also pitched for the varsity baseball team. He obtained a clinical doctorate degree in Physical Therapy from Regis University in 2005.

Wes has advanced certifications in Functional Trigger Point Dry Needling, Active Release Technique and Graston Soft Tissue Mobilization. He has extensive experience treating a wide variety of orthopedic injuries, sports injuries (youth to professional/Olympic) and neuromuscular impairments across age groups and ability levels.

Wes’ clinical interests include, injury rehabilitation and prevention in athletes, overuse injuries in youth athletes and treating sub occipital headache pain. Wes also volunteers his time working with the Colorado Avalanche as well as the Grandview High School and Cherokee Trail High School sports teams.

Outside of work Wes enjoys spending time with his wife Lyndsie, daughter Harper and all varieties of sports.

Andrea entered the Physical Therapy Profession in 2005, after graduating from Regis University with her Doctorate of Physical Therapy. Previous to completing her PT degree she earned a dual Bachelor’s degree from Oklahoma Panhandle State University in Biology and HPER with an Athletic Training emphasis. Andrea has worked in an outpatient orthopedics setting for the past 12 years. Where she has been able to utilize her additional trainings and certifications in Dry needling, ATC (certified Athletic Trainer), and manual therapy skills, including joint manipulations, and soft tissue mobilizations. Andrea and her husband Eric have 3 daughters and enjoy camping together. In her spare time Andrea spends time coaching her daughters in softball and volleyball as well as cheering them on in soccer and karate with her husband.

Ricky Huber, PT, DPT

Ricky is trained in Instrument assisted soft tissue mobilization (ISATM) and specializes in orthopedic therapy. He graduated from Florida International University with a Doctorate in Physical Therapy. For his undergraduate degree he studied at Georgia Tech and achieved a BS in Biomedical Engineering. Ricky enjoys snowboarding, soccer and exploring the great outdoors with his fiancé, Brittany. Ricky has a passion for serving others and strives to help his patients regain their strength and confidence needed to be the best version of themselves.

Taya Skarda, PT, DPT, CSCS, COMT

Taya is a North Carolina native with a passion for any outdoor activity and enjoys working with all age groups of athletes and weekend warriors. Taya has received her doctorate in physical therapy at Regis University and has worked in outpatient orthopedics for 10 years. She takes pride in continued education receiving certifications in dry needling, strength and conditioning, and orthopedic manual therapy. Her current interests are with cervical spine dysfunction and its impact on vestibular symptoms as well as post-concussion syndrome. Outside of work Taya enjoys hiking, rock climbing, snowboarding and traveling with her husband and 5-year-old son. Go TAR HEELS and Broncos!

Kristin Bibbins

Kristin Bibbins is our Front Desk Administrator and has been with Pro Active for two years. She is in charge of scheduling patients, working directly with insurance companies and taking care of the needs of the clinic. Kristin is also a licensed massage therapist who graduated from the Cotiva Institute of Massage Therapy in Arizona in 2014. She massages on Saturday mornings at the clinic, specializing in deep tissue and neuromuscular therapy. When not at work she enjoys yoga, hiking and spending time with her son and husband.

Kevin Cowell, PT, DPT

Kevin is originally from Southern California where he received his Undergraduate Degree at Azusa Pacific University and then continued his education at Regis University in Denver, Colorado where he received his Doctorate of Physical Therapy. As a clinician, Kevin has a passion for orthopedics where he strives to facilitate the care of patients and seek to improve the health and quality of life for the community. Kevin has continued his pursuit for providing optimal care by receiving Certifications in Active Release Techniques (Cert. ART) and as a Vestibular Rehabilitation Specialist (Cert. VRS). In his spare time, Kevin enjoys rock climbing, skiing, and camping in the Rocky Mountains with his family and pup, Dakota. Kevin has been with the Pro Active family nearly 2 years and is excited to continue his care for the Eastern Plains as a Physical Therapist at our location in Strasburg, Colorado.

Daniel Stoot, PT, DPT

Dan is originally from Colorado Springs, Colorado and completed his Doctorate in Physical Therapy at Campbell University in Buies Creek, North Carolina. As a clinician, Dan takes great pride and joy in being a facilitator to those in need and assisting patients on their journey from dysfunction to return to function. When not treating patients Dan loves spending time in the Rocky Mountains fly fishing and camping. Dan enjoys treating a wide variety of orthopedic conditions and is currently working at our location in Strasburg, Colorado.

A native of the California desert, Kate moved to northern Colorado in 2003 and received her Bachelor of Science degree in Exercise Science (Athletic Training) from the University of Northern Colorado in 2006. She earned a Doctorate of Physical Therapy from Regis University in 2011. Kate is a Licensed Physical Therapist and Certified Athletic Trainer. She has been working with the Justin Sportsmedicine Team for the past 12 years, treating acute and chronic injuries for professional rodeo athletes in the Rocky Mountain region. Kate is Full Body Certified in Active Release Techniques and is a Certified Trigger Point Dry Needling Provider. She also works closely with area doctors and schools to manage athlete and adult concussions and has become a Certified Vestibular Rehabilitation Specialist. Kate is a preceptor for the UNC Athletic Training Program, mentoring future certified athletic trainers. She specializes in the treatment of orthopedic injuries, sports injuries, cumulative trauma disorders, occupational injuries, concussions and vestibular disorders. Kate is a member of the American Physical Therapy Association- Colorado Chapter, the National Athletic Trainers’ Association, and the Rocky Mountain Athletic Trainers’ Association. She enjoys the Colorado outdoors, riding horses, snow skiing and spending time with her family and friends.

Kristen received her Doctorate of Physical Therapy degree from the University of Colorado Health Sciences Program in 2008. She graduated with a Bachelor of Science degree in Kinesiology from the University of Northern Colorado in 2004. She is a certified trigger point dry needling provider and has attended additional training in the areas of pediatrics, vestibular evaluation and treatment, kinesiotaping, and manual skills. Her focus is on providing individualized care to her patients utilizing a function-based approach with evidence-based practice. She enjoys working with people of all ages and has extensive experience with pediatrics as well as general orthopedics conditions, injury prevention, neuromuscular disorders, and vertigo. She is a Colorado native who enjoys the outdoors and spending time with her family.

Kaleb Zier PT, DPT
Physical Therapist

Kaleb was born and raised in a small farming community in Basin, Wyoming. At the University of Wyoming he received his Bachelors of Science degree in Kinesiology and later obtained his Doctorate of Physical Therapy from the University of Jamestown in Fargo, North Dakota. In high school he was involved in numerous sports; football and basketball being his favorite. During his time as an athlete he had his fair share of injuries and multiple surgeries that introduced him to the physical therapy world. Kaleb enjoys working with patients of all ages to get back to what they love doing. He is looking forward to progressing his manual therapy skills and getting his Manual Therapy Certifications. On his off days you can find him going to a football game, hiking, and going for random road trips with friends and family.

Erin Amundsen OTR, CHT

Erin received her Bachelor of Science degree in Psychology from Colorado State University in 2004. She received her Master of Science degree in Occupational Therapy from CSU in 2006. She is a licensed Occupational Therapist and Certified Hand Therapist. Erin has practiced in several states and worked internationally as a Registered Hand Therapist in New Zealand. Erin’s clinical interests include: upper extremity trauma, fracture and tendon management, and neurological disorders. She specializes in treatment of the upper extremity and hand, providing services at Pro Active Greeley and Eaton locations. Erin is a Colorado native. She lives in Fort Collins with her husband Mark. She enjoys traveling, sports and local microbrews.

Alan Parker MS, ATC
Certified Athletic Trainer

Alan Parker is a BOC Certified Athletic Trainer who works with athletes to prevent, evaluate, and rehab injuries and medical conditions. He holds a Bachelor’s Degree in Athletic Training from Colorado State University-Pueblo as well as a Master’s Degree in Sports Medicine/Athletic Training from Indiana Wesleyan University. Alan has worked in the high school, university, and clinical setting in his time as a professional ATC. He believes attitude and a positive mindset are key values that go hand-in-hand with traditional medicine and are important in the maintenance of healthy athletes. Alan strives to create lasting relationships, which he finds is vital to patient recovery and well-being. In his free time, Alan enjoys reading comics, playing video games, and watching football. Todd received his undergraduate degree from Colorado State University in Human Nutrition and Dietetics with a minor in Human Anatomy and Neurobiology in 1996. He received a Master’s of Science in Physical Therapy (1998) and later completed his transitional Doctorate of Physical Therapy (2008) from Regis University. He has over 20 years of Clinical Outpatient Orthopedic and Sports Medicine experience. He has served as affiliate faculty in the Department of Physical Therapy program at Regis University. Todd has successfully passed the Orthopedic Specialty Examination through the American Physical Therapy Association. He is also a Certified Strength and Conditioning Specialist through the National Strength and Conditioning Association. He has been certified as an Active Release Technique (ART) provider. He has extensive experience treating a wide variety of orthopedic injuries, sports injuries (amateur to professional levels) and occupational injuries. Todd’s passion is to further develop his skills as a manual-based orthopedic Physical Therapist through a commitment to life-long learning. Dan Goss, OTR, CHT

Dan received his Bachelor of Science in Occupational Therapy from Colorado State University in 1993. He began his career in Lake Havasu City, Arizona where he specialized in hand therapy. In 1999, he joined the United States Navy as a Medical Service Core officer focusing in hand therapy. He spent the next 11 years as a hand therapist, working side by side with 9 board certified hand surgeons. His last duty station was in Washington D/C, where he was in charge of the Occupational Therapy Department at the National Naval Medical Center in Bethesda, Maryland. While there he also was assigned head of the Outpatient hand therapy program at Walter Reed Army Medical Center in Washington, D/C. During the 4 years of his assignment in Washington D/C, he had the opportunity to work with Senator’s, Congressmen, military personnel, their families and military personnel who were injured in the Iraq or Afghanistan war.
Dan has been a hand therapist for over 19 years. During that time, he has made over 5000 custom splints and treated everything from tendonitis, carpal tunnel, upper extremity fractures to burns, gun shoot wounds, IED blast injuries and poly trauma.
Dan is a native of Colorado and has returned home to Northern Colorado with his wife and 2 kids. In January 2013, Dan joined the outstanding team at Pro Active Physical Therapy where his 19 years of hand therapy experience are at work.

Bryana Dacus, DPT
Physical Therapist

Bryana is a Greeley native and is excited to be working in the community she grew up in. She received her Doctorate of Physical Therapy degree from the University of Puget Sound in Tacoma, Washington. She received her bachelor’s degree in Health and Exercise Science with a concentration in Sports Medicine from Colorado State University. Bryana also received a minor in Spanish and grew up in a bilingual household. She has been with Pro Active since 2012 and is passionate about helping people recover from orthopedic injuries and get back to doing what they love. She also has special interests in manual therapy and vestibular rehabilitation. Bryana is a member of the American Physical Therapy Association (APTA) and is committed to providing patient-centered care. Outside of work, she enjoys spending time with her new husband Zane, and staying active at the gym or in the beautiful Colorado sunshine with family and friends.

Rachel is a Physical Therapist Assistant from Ames, Iowa. Dennis Martin, PT, ATC, Cert. ART, Cert. DN
Owner/Director

Dennis is a dual credentialed provider who has more than 20 years of clinical and field experience in athletic training and physical therapy. Dennis earned his degree in Kinesiology with an emphasis in athletic training at the University of Northern Colorado in 1995. He finished his Master’s degree in Physical Therapy from Regis University in 1998, where he was honored with a clinical achievement award and excellence in research. Dennis worked at the Steadman Hawkins clinic for six years, where he experienced countless hours of surgical observation and worked three seasons assisting the Denver Broncos during training camp and game coverage. Dennis is the co-founder of Pro Active Physical Therapy, which has been in business since 2006. Beyond his extensive clinical experience, Dennis has completed hundreds of hours of clinical education with emphasis in Active Release Techniques, spinal manipulation, trigger point dry needling and concussion evaluation and management. Clinically, Dennis has a special interest in postoperative management of the extremities, acute injury evaluation, management of spinal dysfunction and headaches. When Dennis isn’t working he spends his time with his three son, Mitch 20, Conner 18, and Landen 12.

Ryan Palkoner, PT, MPT, Cert ART, Cert DN

Ryan Palkoner is in his 17th year of clinical practice after receiving his Master of Physical Therapy degree from Nova Southeastern University. Ryan’s background consists of advanced training in joint manipulation and soft-tissue mobilization including Active Release Techniques (ART) which has emerged as the Gold Standard in myofascial release. He is a certified McKenzie provider and also holds a certification in Trigger Point Dry Needling (Cert DN) from the Spinal Manipulation Institute. As a clinician, Ryan has extensive experience in outpatient orthopedic settings and has utilized Instrument-Assisted Soft-Tissue Mobilization (IASTM) and its many tools as well as Kinesiology Taping Techniques for over 15 years. Ryan is part of the Ironman
Provider Network and has been part of the medical staff attending several Ironman Triathlons, the AVP Pro Volleyball Tournament and the Carlsbad Marathon which has given him the opportunity to treat a multitude of injuries in numerous professional and recreational athletes.

In addition to being a seasoned and practicing Physical Therapist, Ryan lectures around the country teaching practitioners from a variety of disciplines in the areas of manual release techniques, IASTM and Kinesiology taping. He is a member of the American Physical Therapy Association, Orthopedic Section.

Matt Franzino, PTA

A native of Long Island, NY, Matt has worked with Pro Active Physical Therapy for over 5 years. Starting as a Physical Therapy Aide, Matt has since continued his education at Pima Medical Institute in Denver, CO, where he received his license as a Physical Therapist Assistant in 2016.

Matt has experience in both inpatient and outpatient therapy, having strong interests in manual therapy and orthopedics, while aspiring to be an Active Release Technique (ART) provider in the near future. Clinically, Matt believes therapy should be safe, effective, and efficient, but puts a huge emphasis on educating and building a rapport with his patients.

In 2015, Matt and his wife gave birth to their first child, who he loves spending time with, in addition to exploring Colorado, including both the outdoors and the restaurant scene. Matt also has a passion for golf, and watching his favorite sports teams play.

Dr. Tyler Ready is in his 8th year of practice after receiving his Clinical Doctoral degree from the University of Colorado, School of Medicine. He has gone on to complete his diploma in Osteopractic Physical Therapy through the American Academy of Manipulative Therapy. This is a prestigious diploma that less than 100 Physical Therapists hold throughout the world. This advanced post-doctoral diploma has been a culmination of 5 years of continuing coursework in advanced Spinal and Extremity Manipulation (Cert-SMT), Dry Needling (Cert-DN), differential diagnosis, and treatment for patients with complex spine and extremity conditions. Tyler obtained a certification as a Vestibular Rehabilitation Specialist (VSR) and is a certified Graston provider. Although his clinical interests are broad, he most enjoys treating patients suffering from chronic headaches, complex spine pathologies, and sports and overuse injuries. Tyler is passionate about teaching, and mentors numerous clinical doctorate students each year. That passion has led to speaking engagements to doctoral students on the topic of business entrepreneurship in private practice and leadership in our profession. Outside of the clinic he enjoys spending time with his wife and 2 young daughters, staying involved in the fitness community, and enjoying the beautiful mountains of Colorado.

 

Tyler Pollock SPT

Tyler is originally from Grand Junction, CO and is currently in his final year at the University of Colorado School of Medicine obtaining his Doctorate of Physical Therapy. Prior to physical therapy school, Tyler graduated from Colorado Mesa University with his Bachelor’s degree in Exercise Science. He became interested in the field after recovering from several injuries he sustained while playing baseball in high school and college. He has a strong interest in manual therapy techniques and plans to continue to develop in this area of practice. He is excited to join the Pro Active team beginning in January 2018 and can’t wait to begin his career in a clinic that values patient care and a research based approach. Upon graduation, Tyler will pursue further education in advanced spinal manipulation techniques, dry needling, and biomechanical movement analysis of overhead athletes. In his spare time, he enjoys hiking, camping, recreational sports, CrossFit, and is an avid sports fan for the local Colorado teams.

Dr. Michael “VP” Van Portfliet, PT, DPT, OCS

Dr. Michael “VP” Van Portfliet developed an interest for the human body and it’s potential while participating in multiple competitive sports growing up. His interest developed into a passion while working with rehabilitation professionals after numerous injuries in high school and college. This passion, combined with a desire to work with people, lead him to pursue physical therapy. VP graduated from Regis University with a Doctoral Degree in Physical Therapy and went on to obtain certifications in Advanced Spinal Manipulation (Cert. SMT), Dry Needling (Cert. DN) and Vestibular Rehabilitation (Cert. VRS). In March of 2017, he became a Board-Certified Orthopedic Clinical Specialist (OCS), giving him the opportunity to become a Clinical Instructor for physical therapists pursuing their residency in orthopedics. VP believes in an evidence-based and eclectic approach to physical therapy and strives to empower his patients to be active participants in the management of their own health and performance. He enjoys serving and working with the community in the greater South Denver area. In his spare time, VP enjoys staying physically active by going to the gym and taking advantage of Colorado’s outdoors with his wife, Janelle, and dog, Clyde.
